SERVICE PLAN ELEMENTS

Service Plans are based on an individual’s needs and abilities with the primary focus on health, safety and personal growth.

* SAFETY- Identify emergencies, use 911, self-evacuate, first aid, home and community safety skills.

* MEDICAL- Medical appointments, medication management, and record keeping.

* FINANCIAL– Money handling, bill paying, banking and budgeting.

* DAILY LIVING SKILLS-Personal care and home care.

* NUTRITION – Planning & preparing meals, shopping and safe food storage.

* EDUCATION – Creative daily learning and awareness of formal education opportunities.

* EMPLOYMENT – Support and encourage community-based employment.

* TRANSPORTATION – Teach the use of public transportation and provide transportation as needed.

* RELATIONSHIPS – Support the importance of family and friends. Encourage respect of others.

* COMMUNITY AWARENESS – Provide opportunities for awareness and participation in community events.

ABOUT THE CORPORATION

guide, inc. was organized in 1994 for the primary purpose of providing the services of a Supported Living Program to mentally and / or physically challenged individuals. 

Services are provided within the homes and communities of the programs’ participants. 

Participants and families are provided with a phone number that is available 24 hours a day for emergencies; and 7:00 am to 4pm, Monday thru Friday for all other communications.

The Supported Living Program is designed to provide guidance to the participants and enable independent living. guide, inc. mentors assist and teach individuals 

in the fundamentals of daily living skills as outlined in a service plan. Supports encourage healthy lifestyles including areas of nutrition, active living, clean environments and community participation.

Teams consist of the participant, their mentors, family members, case managers, social workers and medical personnel as needed.

Your Supports & Activities

Your Individualized Support Plan:

Individualized supports mean you matter and it means you won’t be lost in a group of others. You will have a person-al mentor, one that will work with you and provide guidance, teach, and assist you with achieving your goals.

Being independent is more than doing it on your own; it is having your own opinion and hav-ing your own voice to share it. You have the right to be heard and be involved in the decisions of your life. You have the right to a support team that you have chosen. A team com-mitted to helping you develop your personal support plan. You and your team will design your life supports and identify your individual goals. Personal choice includes where you want your supports to occur, when you want supports, and who will provide your supports. You may choose to have supports at home, at work, or in the community. No matter what you choose, your plan will include your hopes, dreams, and goals. It is your Life, it is your Support Plan.

Supports for Home Life:

You start with making choices on who will be part of your Support Team. Then you and your team will develop a plan that fits your needs and goals. You may benefit from supports for personal care, nutrition, medical and emotional care, budgeting and bill paying, applying for benefits, homecare, and even opportunities outside of the home to socialize and make friends, or participate in community events.

Your supports can take place within your family home, at the Guide Training Center, in the community, or at your own home. Supports are scheduled on the days and hours that work best for you. guide, inc. understands that it is your Support Plan because it is your Life! Supports for Work and Community:

Choose your type of supports, it might be volunteering, working, or spending time in the com-munity. Maybe you want to do a little of each, your choice. You and your team will plan your support days, hours, and focus on your interest and goals. You will have a Mentor to provide you with transportation, guidance, and assistance working toward achieving your goals.

Family Style Activities:

Although, Support Plans focus on a one to one approach with just you and your mentor, guide, inc. believes that socialization with peers is part of a healthy lifestyle. You will be invited throughout the year to attend some Family Style gatherings that are a great way to meet up with old friends and make new ones, as well as enjoy some good food and fun.

In June, the Guide Family takes a trip to Quassy Amusement park for rides and a picnic lunch of burgers, fries, and ice cream. Hope you will join us next June!

Guide Family and Friends get together in August for a day and night of Camping fun. There is a different theme each year with team activities and great food. We end the evening with a campfire, smores, and ground fireworks. Truly an amazing day that includes so many activities to choose from.

The final Guide Family event each year is the Holiday Gathering, none-like you have ever attended before. The theme, decorations, group activity, and food change each year. Each year, the new theme is a secret, making it a little exciting and certainly entertaining.

Guide participants and mentors alike, enjoy the Family Style Activities for both entertainment and friendship.
